29 CFR1910.212(a)(1): The employer did not ensure one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks such as but not limited to Line 4 1999 Bekum H-155 MACH #066. Line 4: On or about February 20, 2023, and at times prior to, the employer did not ensure that the Bekum H-155 MACH #066 takeout system was equipped with a guard or equivalent means of protecting the employee from crushing hazards while unjamming a bottle from the nest area resulting in partial finger amputation.
