29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  a) Jobsite at 325 S 18th Street, Philadelphia, PA- On or about March 22, 2023, the employer did not ensure that each working level was fully planked or decked between the front uprights and guardrail supports. An employee engaged in masonry activities was working from a scaffold platform that was not fully planked, thereby exposing employees to fall hazards of approximately 48 feet.

29 CFR  1926.451(c)(1)(ii): Guys, ties, and braces were not installed according to the manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations every 26 feet or less for scaffolds greater than 3 feet wide:  a) Jobsite at 325 S 18th Street, Philadelphia, PA- On or about March 22, 2023, the employer did not ensure the scaffold braces were installed at the 4:1 height and repeated every 26 feet.  An employee engaged in masonry activities was utilizing a scaffold approximately seven stories high that was only restrained from tipping by a brace installed at the top level, thereby exposing employees to fall hazards of approximately 48 feet.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a) Jobsite at 325 S 18th Street, Philadelphia, PA- On or about March 22, 2023, the employer did not provide a ladder, stairway, or other means of access to scaffold platforms.  An employee engaged in masonry activities was utilizing the scaffold frame to access platforms up to the seventh level, thereby exposing employees to fall hazards of approximately 48 feet.

An employee engaged in masonry activities was utilizing a scaffold platform that did not have guardrail systems along the open sides and ends of the working platform without the use of fall protection, thereby exposing employees to fall hazards of approximately 48 feet.
