Clearfield Leather DBA Wickett and Craig of America, Inc; 120 Cooper Road Curwensville, Pa 16833, on or about 11/22/2022 and at times prior thereto: Employees were exposed to a 20-foot fall hazard located at the back of the elevator platform. Employees regularly were within 2' of an unprotected 20' fall hazard while transferring material off the elevator.

The missing cover exposes employees to hazards including but not limited to, electrical shock and malfunctions associated with electrical connections exposed to wet conditions.
