29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service: a.) Clearfield Leather DBA Wickett and Craig of America, Inc; 120 Cooper Road Curwensville, Pa 16833, on or about January 15th, 2025 and at times prior thereto: Employees in the Beam House area were exposed to hazards associated with an open cover/door on the Yale Tow Truck (Serial # B903N01926L). The open cover/door exposed employees to hazards including but not limited to, electrical shock and malfunctions associated with electrical connections exposed to wet conditions. b.) Clearfield Leather DBA Wickett and Craig of America, Inc; 120 Cooper Road Curwensville, Pa 16833, on or about January 15th, 2025 and at times prior thereto: The nameplate on the Yale Tow Truck (Serial # B903N01926L) located in the Beam House area was not legible. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent: a.) Clearfield Leather DBA Wickett and Craig of America, Inc; 120 Cooper Road Curwensville, Pa 16833, on or about January 15th, 2025 and at times prior thereto: The Yale Tow Truck (Serial # B903N01926L) was defective in that the cover/door was open exposing live electrical connections. The Yale Tow Truck was defective and not tagged out of service. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
