29 CFR 1926.416(e)(1):Worn or frayed electric cords or cables were used: a) 13 Kind David Dr., Lakewood, NJ 08701: Employees were exposed to shock hazards associated with frayed extension cords. The extension cords used by employees had multiple frayed areas and were repaired incorrectly with electrical tape. The cords were used to power circular saws and other equipment. Observed on or about 03/30/23. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) 13 Kind David Dr., Lakewood, NJ 08701: Employees were observed working at a height of approximately 17 feet 4 inches without fall protection. Personal fall protection system and/or a guardrail system were not used when working on and near the unprotected edge of the second floor of the new construction. Observed on or about 3/30/23. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1051(a):A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a) a) 13 Kind David Dr., Lakewood, NJ 08701: Employees were exposed to falls of 11' 8" to concrete floor below. Employees were using angled 2x4s and frames to climb from the first floor to the second level of the building. The employees used the same method to come down from the second floor. Adequate ladders were not provided or required by the employer. Observed on or about 03/30/23. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
