29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   At the jobsite, employees operating Interchange Hitachi nail guns while performing roofing activities were not wearing eye protection, exposing employees to the hazard of being struck by flying objects.  Miguel Landaverde Gallegos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1572630, Citation Number 1, Item number 2, and was affirmed as a final order on  March 28, 2022, with respect to a workplace located at 2935 South Griffins Lane, HEARTLAND, TX 75126.  Miguel Landaverde Gallegos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1501053, Citation Number 1, Item number 1, and was affirmed as a final order on  February 21, 2021, with respect to a workplace located at 809 Lauren Grove Lane, FORNEY, TX 75126.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   At the jobsite, employees engaged in roofing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 11 feet to the ground below. At the residential construction site, employees accessing the roof on a metal ladder with side rails that did not extend at least 3 feet above the edge of the roof were potentially exposed to fall hazard greater than 11 feet. 29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.   At the residential construction site, the employer did not ensure employees performing roofing activities were trained to recognize and avoid slip and fall hazards related to ladders.
