29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) On the roof of the residence: On or about 4/6/2023, employees were exposed to fall hazards without an appropriate method of fall protection, while working on an approximate 10/12 pitch residential roof, approximately 18 feet above the lower level.

29 CFR  1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee.   (a) At the worksite: On or about 4/6/2023, employees were exposed to fall hazards while working on a residential roof, approximately 18 feet above the lower level, by utilizing improperly installed hinged roof anchors, which were missing nails/screws, and had more than one employee lanyard attached to it, making it incapable of supporting at least 5,000 pounds per employee.
