Jobsite: 109-15 S. 24th St. Philadelphia, PA 19103 - On or about April 6, 2023, the employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during framing activities, such as but not limited to fall hazards.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section.  Jobsite: 109-115 S. 24th St. Philadelphia, PA 19103 - On or about April 6, 2023, the employer did not ensure that employees conducting framing operations from the 4th level of a residential building were protected from falls by use of guard rails or personal fall restraint systems.  a) One employee from the top of the unfinished 4th level, breaking the vertical plane was exposed to falls of approximately 40 feet.  b) Two employees working from a plank attached to the exterior wall of the 4th level.,  was exposed to falls of approximately 34 feet
