29 CFR  1910.147(c)(4)(ii)(B): Specific procedural steps were not developed for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:   (a)Trail King Industries, Inc. at 1200 Gehl Drive, Yankton, SD 57078: On or about and at times prior to March 30, 2023, the employer failed to develop machine specific procedural steps for shutting down, isolating, blocking and securing machines or equipment with multiple energy sources. The Flex CNC G-Series Vertical Machining Center and Adhesive machine were equipped with multiple energy sources and specific procedural steps were not developed to control hazardous energy. This condition exposed employees to the hazards of improper lockout.  Abatement Note:  The procedures shall include but are not limited to the following:  (1)	A specific statement of the intended use of the procedures;  (2)	Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  (3)	Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and  (4)	Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(c)(7)(i)(B):  The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees, and to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:  (a)Trail King, Industries, Inc., at 1200 Gehl Drive, Yankton, SD  57078:  On or about and at times prior to March 30, 2023, the employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees, and to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices, including but not limited to the following machines or equipment:  (1)	Flex CNC G-Series Vertical Machining Center; (2)	Accupress Advantage; (3)	Adhesive machine  Abatement Note:  The training shall include the following:  (1)	Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  (2)	Each affected employee shall be instructed in the purpose and use of the energy control procedure.  (3)	All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)Trail King Industries, Inc. at 1200 Gehl Drive, Yankton, SD 57078: On or about and at times prior to March 30, 2023,  the employer did not ensure an employee was protected from laceration and amputation hazards associated with using the Hydmech horizontal bandsaw in the fabrication department. An employee was using the horizontal bandsaw to cut parts for a wall stand while not moving the guard to the size of the stock being cut and cover the 7 inches of unused blade. The condition exposed an employee to laceration and amputation hazards.
