29 CFR 1926.1053(b)(11): Ladder(s) was/were moved, shifted, or extended while occupied: a- 218 - 223 Stanton Road, Mount Holly NJ: On or about 06/06/23 the company worksite Foreman was exposed to fall hazards, up to and in excess of 12 feet to the ground surface below, when occupying the ladder's lower rungs while extending an aluminum extension ladder in order to lower its extended fly section. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall: a- 218 - 223 Stanton Road, Mount Holly NJ: On or about 06/06/23 a company worker was exposed to fall hazards, up to and in excess of 25 feet to the ground surface below, when climbing up an aluminum extension ladder while carrying a card board box, approximately six foot in length, with one of their hands used to climb the ladder. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

As such, these workers were exposed to fall hazards up to and in excess of 25 feet to the ground surface below.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. a- 218 - 223 Stanton Road, Mount Holly NJ: Workers were exposed to fall hazards, in excess of 25 feet to the ground surface below, when accessing and egressing the pitched roof of a residential structure using an aluminum extension ladder that contained broken rungs, bent side rails, broken feet, missing feet, etc. Condition observed on or about 06/06/23.
