The citation was issued on 06/07/2024 and received by the employer on 06/20/2024.  The citations became a final order on 07/03/2024.  Violation occurred on or about 02/14/2025.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)   On or about: February 2, 2024 Location: 52-62 Willow Lane, Parsippany, NJ 07054  a) Employees engaged in roofing activities on the 6 on 12 pitched roof of a two story residential building were exposed to fall hazards of approximately 20-feet and were not trained to recognize and mitigate fall hazards by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about: February 2, 2024 Location: 52-62 Willow Lane, Parsippany, NJ 07054  a) Employees accessed a roof approximately 20-feet from the ground below from a fully extended portable ladder whose side rails extended only about 18-inches above the roof, an upper landing surface. The subject ladder was not secured at its top to a rigid support surface that will not deflect.

29 CFR  1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:   On or about: February 2, 2024 Location: 52-62 Willow Lane, Parsippany, NJ 07054  a) Employees were utilizing a portable aluminum extension ladder that was placed on unstable ground due to the soft ground. Extension ladders used on surfaces that were not stable and level were not secured to prevent accidental displacement.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about: February 2, 2024 Location: 52-62 Willow Lane, Parsippany, NJ 07054  a) Employees engaged in roofing activities on a 6 on 12 pitched roof using pneumatic nail guns were exposed to eye loss/eye injuries due to struck by hazards as eye protection was not being utilized.  The Dynamic General Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1632443, citation number 1, item number 1 and was affirmed as a final order on March 28, 2023, with respect to a workplace located at 62 Senator's Way Berlin NJ 08009.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system:  On or about: February 2, 2024 Location: 52-62 Willow Lane, Parsippany, NJ 07054  a) Employees engaged in roofing activities on the 6 on 12 pitched roof of a two story residential building  were exposed to fall hazard of approximate 20-feet to the ground below as a personal fall arrest system, guardrail system, or safety net were not being used.  The Dynamic General Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1675156, citation number 2, item number 1 and was affirmed as a final order on January 31, 2024, with respect to a workplace located at 218 - 223 Stanton Road Mount Holly, NJ 08060.
