On or about June 8, 2023 and at times prior thereto, 11A re-grinder area, automotive parts, a white relocatable power cord, cardboard boxes, and debris on the floor exposed employees to tripping hazards. b. On or about June 8, 2023 and at times prior thereto, 11A re-grinder area, building ledges and elevated surfaces had plastic dust accumulations which exposed employees to combustible dust fire and explosion hazards.

29 CFR 1910.305(b)(2)(i):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose. On or about June 8, 2023 and at times prior thereto, at the exterior 11A Re-grinder area, a cover was not provided on an electrical junction box which contained conductors for a re-grinder. This exposed employees to electrical shock hazards.
