29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:  a)  On or about July 5, 2023, employees working from frame scaffolding were exposed to fall hazards of approximately 28 feet.  North South Masonry & Tuckpointing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1) which was contained in OSHA inspection number 1446212, citation number 1, item number 1b, and was affirmed as a final order on December 5, 2019, with respect to a workplace located at 2732 N Kimball Ave, Chicago, IL 60647.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about July 5, 2023, an employee was performing chimney work from a roof while exposed to fall hazards of more than 28 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
