29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: At a worksite at 101 S Dean Ave, Raymore, MO 64083: Workers were exposed to struck-by hazards. This was last demonstrated on or about June 29, 2023, when employees were permitted to operate pneumatic nail guns at 70 to 120 psi without wearing impact-resistant eye protection while roofing a residential building.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At a worksite at 101 S Dean Ave, Raymore, MO 64083: Workers were exposed to fall hazards. This was last demonstrated on or about June 29, 2023, when workers were engaged in roofing activities of a residential building. Workers placing roof decking were exposed to fall hazards of approximately 30 feet from the eve of the roof to the ground below. No guard rails, safety net system(s), or personal fall arrest system(s) were utilized to prevent falls.
