The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a. On or about July 17, 2023, at the above addressed jobsite, an employee conducting residential roofing operations was not wearing personal fall arrest system (PFAS) with the attachment point at the employee's back, at a fall height for twenty-three (23) foot eight (8) inches above the ground, thereby exposing the employee to the hazards associated with falls.

On or about July 17, 2023, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall hazards, thereby exposing the employees to the hazards associated with falls.
