29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) Job site, 1600 Termon Avenue, Pittsburgh, PA: On or about August 19, 2023, during construction activities, the employer did not ensure that scaffolding was inspected by a competent person for unsafe conditions. Deficiencies included, but were not limited to, lack of fall protection in some scaffold areas, reliance on either a single rail section for fall protection at the end of scaffolding or a single cross brace for fall protection along the rear side of the scaffolding, working level not being fully planked and having electric lines that ran through the scaffolding and were not shut off or covered with insulative materials.
