29 CFR 1926.451(e)(9)(i): The employer did not provide a safe means of access for each employee erecting or dismantling a scaffold where the provision of safe access was feasible and did not create a greater hazard:   a) Job site, 609 Bower Hill Road, Bridgeville, PA: On or about October 17, 2024, during scaffold erection activities, the employer did not ensure that a safe means of access was provided when an employee was getting on/off the scaffold in that he climbed the scaffold framing to get up/down.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Job site, 609 Bower Hill Road, Bridgeville, PA: On or about October 17, 2024, during scaffold erection activities, the employer did not ensure that an employee wore head protection at ground level while handing up scaffolding components to an employee on the scaffold above him.  Reber Masonry & Restoration LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.100 (a), which was contained in OSHA inspection number 1692111, citation number 1, item number 1, and was affirmed as a final order on October 1, 2023, with respect to a workplace located at 1616 Chateau Street, Pittsburgh, PA.

29 CFR 1926.451(g)(2): The employer did not have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds or did not require fall protection to be used for employees erecting or dismantling supported scaffolds where the installation and use of such protection was feasible and did not create a greater hazard:    a) Job site, 609 Bower Hill Road, Bridgeville, PA: On or about October 17, 2024, during scaffold erection activities, the employer did not ensure that employees used appropriate fall protection systems/devices where exposed to falls of over 10 feet.   Reber Masonry & Restoration LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(g)(2), which was contained in OSHA inspection number 1692111, citation number 1, item number 2, and was affirmed as a final order on October 1, 2023, with respect to a workplace located at 1616 Chateau Street, Pittsburgh, PA.

29 CFR  1926.451(f)(7): Scaffolds shall were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:   a) Job site, 609 Bower Hill Road, Bridgeville, PA: On or about October 17, 2024, the employer did not ensure that the scaffold erection activities were done under the supervision of a competent person. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection or provide a safe way on/off the scaffold, not wearing hardhats, having scaffolding too close to electrical services and working immediately next to, and sometimes in, the main road with no safety signage, no high visibility clothing being worn and only a single reflective road device in use near the scaffold.
