29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Jobsite: 773 East Ashland Ave. Folcroft, PA 19032 - The employer failed to assure effective protections against falls as required by the standard, as demonstrated on or about 12 September 2023, when Employee #1 was installing trusses to build a roof on a building and exposed to fall hazards of approximately 10 feet.  The employee was not wearing a fall protection system to prevent from falling.   Level Edge Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(15) which was contained in OSHA inspection number 1567902, citation number 2, item number 2 and was affirmed as a final order on June 28, 2022, with respect to a workplace located at 411 Main Street Luzerne, PA 18709.  Level Edge Construction, LLC was also previously cited for a violation of this occupational safety and health standard 1926.501(b)(1) which was contained in OSHA inspection number 1339742, citation number 1, item number 1 and was affirmed as a final order on October 18, 2018, with respect to a workplace located at 615 Memorial Parkway Phillipsburg, NJ 17579.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:  a) Jobsite: 773 East Ashland Ave. Folcroft, PA 19032 - On or about September 12, 2023, the employer did not provide a training program for each employee who was working on the roof of a one story building, installing trusses in close proximity to unprotected sides and edges without fall protection, in the recognition and avoidance of fall hazards, thereby exposing employees to fall hazards.  Level Edge Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(1) which was contained in OSHA inspection number 1567902, citation number 1, item number 5 and was affirmed as a final order on June 28, 2022, with respect to a workplace located at 411 Main Street Luzerne, PA 18709.  OR IN THE ALTERNATIVE  29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) Jobsite: 773 East Ashland Ave. Folcroft, PA 19032 - On or about September 12, 2023, the employer did not provide a certification of training for each employee in the recognition and avoidance of fall hazards.
