29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 1775 Welsh Road Mohnton, PA - On or about April 16, 2025, employees were exposed to a fall hazard of approximately 8 feet to the roof below and 15 feet to grade below the roof's eave due to the ladder on the roof used to access the second roof tier not being extended 3 feet above the landing.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50 feet (15.25 m) or less in width, each employee was not protected by the use of a monitoring system.  a) 1775 Welsh Road Mohnton, PA - On or about April 16, 2025, an employee was observed performing roofing activity at the ridge on a low-slope roof without the benefit of a fall protection system, exposing employees to a fall of approximately 30 feet to the grade below.  b) 1775 Welsh Road Mohnton, PA - On or about April 16, 2025, employees were observed performing roofing activity on a low-slope roof without the benefit of a fall protection system, exposing employees to a fall of approximately 15 feet to the grade below.  Level Edge Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(15), which was contained in OSHA inspection number 1567902, citation number 2, item number 2 and was affirmed as a final order on June 28, 2022, with respect to a workplace located at 411 Main Street Luzerne PA 18709.  Level Edge Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1697039 , citation number 1, item number 1, citation number 1, item number 2, number 1, item number 3, and number 1, item number 4, and was affirmed as a final order on April 1, 2022, with respect to a workplace located at 773 East Ashland Ave Folcroft, PA 19032.  Level Edge Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1714511, citation number 1, item number 1 and was affirmed as a final order on April 2, 2024, with respect to a workplace located at 1300 12th St Eddystone, PA 19022.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
