1926.651(j)(1)  Adequate protection shall be provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face. Such protection shall consist of scaling to remove loose material; installation of protective barricades at intervals as necessary on the face to stop and contain falling material; or other means that provide equivalent protection. 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).   (a) On or about October 2, 2023, at the worksite located at 2303 North Mayfair Road, Wauwatosa, WI 53226, the employer did not ensure that an adequate protective system was used in a trench to protect employees from potential trench collapse hazards. Specifically, on the north end of the trench box  where an employee was at the edge of the trench box to connect water main valve, the vertical wall of the north end of the trench was not protected from cave-in of soil, exposing the employee to a cave in and crushing hazard.
