On and before October 2, 2023, with respect to an establishment located at 4704 Bayberry Street, WI 54476, the employer did not ensure that employees used eye wear equipped with splash protection when working with, and dispensing, substances such as Methyl Ethyl Ketone and Methyl Amyl Ketone.

On and before October 2, 2023, with respect to an establishment located at 4704 Bayberry Street, Schofield, WI 54476, the employer did not ensure that employees performing service and maintenance tasks inside of the paint area of Facility 6 were affixing locking devices in a manner that would hold them in the "off" position when employees were leaving keys in the locks as they performed the service and maintenance tasks.

On and before October 2, 2023, with respect to an establishment located at 4704 Bayberry Street, Schofield, WI 54476, employees were operating a Raymond Model 20-R30TT Reach Truck with a Wesco Industrial Products, Inc Drum Grab and did not have access to an instruction plate that covered the capacity, operation and maintenance of the lift with the attachment.
