JOB SITE: Lot 456, Cardow Dr. & Townshed Way, O'Fallon, MO. On or about 10/5/2023, the employer did not ensure employees used appropriate eye or face protection while using pneumatic nail guns while installing roof shingles. The workers were exposed to a struck-by hazards from flying particles.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were an employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   JOB SITE: Lot 456, Cardow Dr. & Townshed Way, O'Fallon, MO. On or about 10/5/2023, the employer did not ensure employees engaged in roofing activities 6 feet (1.8m) or more above the lower levels were protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.  The employees were not wearing a fall arrest system while installing a roof on a residential building.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  JOB SITE: Lot 456, Cardow Dr. & Townshed Way, O'Fallon, MO. On or about 10/5/2023, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more were extended above the upper landing. Workers performing residential construction were exposed to falling approximately nine (9) feet while using a straight ladder to ascend/descend from the ground level to the roof.

JOB SITE: Lot 456, Cardow Dr. & Townshed Way, O'Fallon, MO. On or about 10/5/2023, the employer did not ensure portable ladders that were broken were immediately marked  in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and withdrawn from service until repaired.
