Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On October 4, 2023, employees of R Garcia LLC were installing shingles on a residential building located at 108 Peruque Estates in Wentzville, Missouri. The employer did not ensure employees performing roofing work at heights utilized fall protection. employees were working on the roof of the building approximately 20 feet above the lower level. The employees were exposed to fall hazards to the ground below.

The employer did not ensure employees climbing ladders did not carrying heavy objects or equipment. While climbing an extension ladder an employee was exposed to fall hazards while carrying a tool belt, air hose, nail gun, and couch cushion.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On October 4, 2023, employees of R Garcia LLC were installing shingles on a residential building located at 108 Peruque Estates in Wentzville, Missouri. Pneumatic nail guns were being used by employees on the Eastside of the roof without donning eye protection. The employees were exposed to struck-by hazards. R Garcia LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1633953, citation number 1, item number 2 and was affirmed as a final order on 5/2/2023, with respect to a workplace located at 652 Carlyle Avenue in Belleville, Illinois.
