OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees are exposed to struck by hazards: a. MSC Walbridge Coatings Inc. located at 30610 East Broadway, Ohio 43465: On or about October 17, 2023, the employer did not ensure the Telescoping Coil Grab #3 (below the hook lifting device) used on the P & H 27 Ton Cab Controlled Bridge Crane, MSC-004, maintained a rated load capacity label. Employees tasked with handing steel coils with this device without the benefit of capacity identification were exposed to struck by and crushing hazards. b. MSC Walbridge Coatings Inc. located at 30610 East Broadway, Ohio 43465: On or about October 17, 2023, the employer did not maintain load testing reports for Telescoping Coil Grab #1 and #3. Employees utilizing the bridge crane equipped with with these telescoping coil grabs without the benefit of capacity testing were exposed to struck by and crushing hazards while handling coiled steel.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. MSC Walbridge Coatings Inc. located at 30610 East Broadway, Ohio 43465: On or about October 17, 2023, the employer failed to ensure the rotating spindle on the rewind was adequately guarded. Employees performing inspection and observation activities on the north side of the inspector's deck at or near the rotating spindle were exposed to caught-in hazards.
