29 CFR  1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces.  MSC Walbridge Coatings, Inc. - Walbridge, Ohio: On or about August 26, 2025, employees were exposed to slip/fall hazards when climbing support structure and machine components at Work Tank #1. The employer did not provide a safe means of access and egress to employees who performed repair tasks from the tank.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed.  On or about August 26, 2025, a portable step ladder was not used as intended in that it was not used in an open position with the metal spreader locking device in place, and was used as a straight ladder to access an upper landing surface on the side of Work Tank #1.  Employees were exposed to falling about 90-inches from the ladder while performing maintenance work.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i).  MSC Walbridge Coatings, Inc. - Walbridge, Ohio: On or about August 26, 2025, the employer did not ensure that employees who accessed Work Tank #1 were protected from fall hazards.  Employees who accessed the tank to repair piping were exposed to fall hazards of approximately 5-feet and up to 14-feet.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
