29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Location: Machine Shop Floor  On or about 11/7/23, the employer did not ensure that machines were guarded at the point of operation, which would protect employees from hazards created by the exposed point of operation, including, but not limited to the following instances:  a) An employee operating the Alpha 550S open horizontal lathe was exposed to struck by and caught in/between hazards created by the exposed point of operation. The spindle operated at high speeds, and the door interlock did not stop the spindle from spinning when opened.  b) An employee operating a CNC lathe, Machine 64, was exposed to caught in-between hazards from a conveyor belt that the employee was observed opening the chute cover and utilizing pliers to remove the metal from the end of the chute.
