29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: 420 Hartford Turnpike, Shrewsbury, MA: On or about November 20, 2023, during a sewer improvement project, the employer did not ensure that the east-facing trench wall was shielded to protect an employee from cave-in and engulfment hazards while working in a trench measured at approximately 12.5 feet in depth.
