29 CFR 1910.95(d)(3):Repeat employee exposure monitoring was not performed whenever a change in production, process, equipment or controls increases noise exposure: HBD/Thermoid, Inc. located at 1301 West Sandusky Street in Bellefontaine, Ohio: On or about November 29, 2023 the employer did not ensure personal noise sampling was repeated when a change in production, process, equipment or controls increased the noise exposures to the extent that additional employees may be exposed at or above the action level. Employees were exposed to hazards associated with elevated noise levels in that in June 2023 the company extended employee work shifts from eight workdays to ten-hour work days.

29 CFR 1910.95(g)(6):At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : HBD/Thermoid, Inc. located at 1301 West Sandusky Street in Bellefontaine, Ohio: On or about November 6, 2023, the employer did not maintain a audiometric testing program by making audiometric testing available annually for instances such as, but not limited to: Instance A On December 5, 2023 an employee assigned as a lead operator float was exposed to noise at 70.04% of the permissible daily dose as measured over 533 minutes of sampling on a 10 hour shift. This dose is equivalent to an 8-hour TWA exposure of 87.4. This dose is also equivalent to an 10-hour TWA exposure of 86.7 dBA accounting for the period of 67 minutes unmonitored. The adjusted action level for a 10 hour shift is 83.7 dBA . Instance B On December 5, 2023, an employee assigned as lead operator of line 1 was exposed to noise at 65.35 % of the permissible daily dose as measured over 527 minutes of sampling on a 10-hour shift. This dose is equivalent to an 8-hour TWA exposure of 86.9 dBA. This dose is also equivalent to an 10-hour TWA exposure of 86.2 dBA accounting for the period of 73 minutes unmonitored. The adjusted action level for a 10-hour shift is 83.7 dBA.
