29 CFR  1910.28(b)(2)(i): 29 CFR 1910.28(b)(2)(i): The employer did not ensure that each employee in a hoist area is protected from falling 4 feet (1.2 m) or more to a lower level by one of the systems listed in 29 CFR 1910.28(b)(2)(i)(A)-(C).  On or about December 29, 2023, at the facility located at 3805 North Freedom Road, Appleton, Wisconsin; the employer failed to ensure that when employees were receiving materials at  the mezzanine in Packaging that they were protected from falls into the hoist area opening.

29 CFR 1910.147(e)(2)(i):   The work area shall be checked to ensure that all employees have been safely positioned or removed.  On or about February 16, 2024, at the facility located at 3805 North Freedom Road, Appleton, Wisconsin; an employee was exposed to machine hazards associated with moving parts while assisting another employee in sanitation on Molder #3.  The one employee was exposed to hazardous machine movement while assisting, and the other employee was using the controls to move the ram.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to sanitize the equipment.  The machine was not deenergized prior to the employee engaging in servicing/maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard  On or about May 2, 2024, at the facility located at 3805 North Freedom Road, Appleton, Wisconsin; the employer failed to ensure employees exposed to injurious corrosive chemicals in the HTST Room were provided with suitable facilities for quick drenching or flushing of the eyes and body, exposing employee to potential chemical burns during sanitation of equipment using Hydroxysan Plus and Dura Foam No 263.

On or about December 15, 2023, at the facility located at 3805 North Freedom Road, Appleton, Wisconsin; the employer failed to ensure the junction box (1HB-2, 4, 6) opening for the conductors from the S.P.E. Electronica Industriale (Model No.: GREENX48-300, Serial No.: 2930157) battery charger was effectively closed.
