29 CFR 1910.147(e)(2)(i):   The work area shall be checked to ensure that all employees have been safely positioned or removed.  On or about February 16, 2024, at the facility located at 3805 North Freedom Road, Appleton, Wisconsin; an employee was exposed to machine hazards associated with moving parts while assisting another employee in sanitation on Molder #3.  The one employee was exposed to hazardous machine movement while assisting, and the other employee was using the controls to move the ram.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to sanitize the equipment.  The machine was not deenergized prior to the employee engaging in servicing/maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
