Rehoboth Boulevard, Milford, DE 19963: On or about January 9, 2024, CSHO witnessed the main entrance (emergency exit) of the building being locked without the panic bar/crash bar able to open the emergency exit without special knowledge and/or tools to open the door.

Rehoboth Boulevard, Milford, DE 19963: On or about January 9, 2024, employees used the Safeline X-ray detector SL2000 machine to assist in the processing of poultry products that did not have signs to protect employees from amputation, electrical shock, guarding, lock out tag out procedure use, and electrical schematic signs, exposing employees to numerous hazards associated with the lack of the signs.
