29 CFR 1910.178(o)(5): The carriage or blades of the industrial truck was not placed under the load as far as possible in order to stabilize the load. a) 255 N. Rehoboth Blvd., Milford, DE 19963 (Rear Yard): On or about August 8, 2024, an employee operating a Linde Power Industrial Truck (PIT), Model H50 EVO, Serial number H2X394J028682018 did not place the forks entirely under the load while moving it. The load was being bulldozed using the forks partially under the load causing the left fork to break free striking the employee in the lower leg resulting in a hospitalization.

Rehoboth Blvd., Milford, DE 19963 (Rear Yard): On or about August 8, 2024, employees were exposed to struck by hazards when a Linde Power Industrial Truck (PIT), Model H50 EVO, Serial Number H2X394J028682018 was used to move a 30-yard dumpster that was not stable or arranged in a manner to prevent equipment damage or falling objects. One employee was struck by a fork that broke free from the forklift.
