29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Vehtek Systems Inc. located at 2125 Woodbridge Blvd. Bowling Green, Ohio, 43402: On or about January 19, 2024, the employer failed to ensure that the in going nip point created between the running conveyor and "conveyor guiding" of unloading conveyor of the 800 ton press in Cold Stamp Department were adequately guarded. Employees stacking and unloading parts of the conveyor in proximity of the unguarded nip point were exposed to caught in hazards.

29 CFR 1910.147(c)(6)(ii): The periodic inspection certification of energy control procedures did not contain the required documentation on identification of the machine or equipment, inspection date, employees included, and the person performing the inspection: a) Vehtek Systems Inc. located at 2125 Wood-bridge Blvd., Bowling Green, Ohio, 43402: On or about January 19, 2024, the employer failed to certify that periodic inspections were performed of energy control procedures of machines such as, but not limited to, the 800 ton press in the Cold Stamp Department, exposing employees to caught in and pinch point hazards while performing maintenance and servicing activities.
