29 CFR 1910.332(b)(1):Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a.) Vehtek Systems Inc, on or about April 15, 2024, at the address 2125 Wood-Bridge Blvd. Bowling Green, OH: The employer did not adequatly train employees on safety related work practices relating to work performed on 240v, and 480v electrical systems. Without the benefit of this training, employees tasked with service and maintenance on these electrical systems were not equipped with the information necessary to avoid exposure to arc flash and other electrical hazards.

29 CFR 1910.335(a)(1)(i):Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: a.) Vehtek Systems Inc, on or about April 15, 2024, at the address 2125 Wood-Bridge Blvd. Bowling Green, OH: The employer did not provide appropriate arc flash rated PPE to employees who regularly worked where 240v, and 480v electrical hazards were present. Without the benefit of arc flash rated PPE, employees tasked with verifying zero energy within open electrical panels and potentially live conductors were exposed to arc flash hazards.
