Employees performing activities such as, but not limited to, die changeouts on the press without the benefit of effective energy isolation procedures were exposed to crushing and pinch point hazards.

Without the benefit of the aforementioned training employee were not equipped with the information necessary to identify and avoid energized parts of the machine during die changeovers.
