29 CFR 1910.184(c)(6): Slings were not securely attached to their loads: Vehtek Systems, Inc. - Bowling Green, Ohio: On or about March 10, 2025, the employer did not ensure that wire-rope slings were securely attached to the 5,150 lb. RH Check Fixture (1801056600) prior to lifting. Employees who used six wire-rope slings to lift the fixture were exposed to struck-by and crushing injuries in that the lifting devices were not attached in a manner that would prevent the material from being displaced and/or falling. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
