29 CFR  1910.184(c)(6): Slings were not securely attached to their loads:  Vehtek Systems, Inc. - Bowling Green, Ohio: On or about March 10, 2025, the employer did not ensure that wire-rope slings were securely attached to the 5,150 lb. RH Check Fixture (1801056600) prior to lifting.  Employees who used six wire-rope slings to lift the fixture were exposed to struck-by and crushing injuries in that the lifting devices were not attached in a manner that would prevent the material from being displaced and/or falling.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s):  Vehtek Systems, Inc. - Bowling Green, Ohio: On or about March 10, 2025, the employer did not ensure that employees were kept clear of loads about to be lifted, and of suspended loads, when using an overhead crane and wire-rope slings to lift the RH Check Fixture (1801056600) in the CMM lab.  Employees were exposed to injury in the event of material displacement and/or falling material.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR  1910.184(f)(1): Wire-rope slings did not  have permanently affixed and legible identification markings as prescribed by the manufacturer  that indicated the recommended safe working load for the type(s) of hitch(es) used, the angle upon which it is based, and the number of legs if more than one:  Vehtek Systems, Inc. - Bowling Green, Ohio:  On or about March 10, 2025, two wire-rope slings used to lift a RH Check Fixture in the CMM lab were not equipped with permanently affixed identification to include information such as, but not limited to, the sling's safe working load for the type of hitch used, the angle upon which it is based, and number of legs. 29 CFR  1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer:  Vehtek Systems, Inc. - Bowling Green, Ohio: On or about March 10, 2025, employees were exposed to injuries associated with falling or displaced loads in that wire-rope slings were not inspected by a competent person each day, prior to use.  Wire-rope slings used in the CMM lab to lift an RH Check Fixture were damaged, not marked with rated capacities, and/or were without identifiable information.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
