29 CFR 1910.253(e)(5)(v): Welding, Cutting, Brazing: Hose showing leaks, burns, worn places, or other defects rendering it unfit for service were not repaired or replaced. Fabrication Shop: On or about February 05, 2024, compressed gas cylinders used for welding, cutting, and/or brazing purposes were fitted with damaged/worn hoses in that the hoses were separating from the brass fittings connecting them to the pressure regulators.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy. Fabrication Shop: On or about February 05, 2024, energized internal electrical parts were exposed in that, in the Grinding Room, two 110 V junction/outlet boxes were missing their cover/face plates. In lieu of the metal faceplates were strips of decaying, blue duct tape.

29 CFR 1910.101(b): Compressed Gases: The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: On or about February 05, 2024, compressed gas cylinders of various content and size, were not properly secured and/or utilized their protective caps for valve protection thereby ensuring that the compressed gas tanks were properly secured and protected from damage. 1) Fabrication Shop: Compressed gas cylinders containing acetylene were stored without protective caps and were not secured to prevent them from being knocked over. (REFERENCE CGA P-1-1965 Sections 3.2.1 and 3.3.8) 2) Fabrication Shop: Compressed gas cylinders containing argon were not secured to prevent them from being knocked over. (REFERENCE CGA P-1-1965 Section 3.3.8) Eagle Manufacturing Company Incorporated, was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1382447, citation number 1, item number 2, and was affirmed as a final order on September 16, 2019, with respect to a workplace located at 13 Homonick Road, Colchester, CT 06415.
