29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. (a) On or about February 6, 2024 the employer did not ensure that employees who performed service and maintenance tasks on the Carbon Fill Station were adequately trained on the means and methods needed to effectively isolate and control hazardous energy sources. Employees were exposed to the potential for unexpected start-up, energization and/or release of stored energy which could result in caught-on, crushing and amputation hazards from the rotary valve while clearing jams/clogs on the Carbon Fill Station. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about February 6, 2024, the employer did not ensure that employees implemented energy control application steps prior to allowing them to reach in the lower hopper on the Carbon Fill Station to remove a blockage. The machine was not shut down or turned off prior to the employees engaging in this servicing and maintenance activity, exposing them to amputation hazards from the unexpected movement of the rotary valve when they reached through the flange to clear a blockage. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
