29 CFR 1904.29(b)(1): The log of all recordable work-related injuries and illnesses (OSHA Form No. 300 or its equivalent) was not completed in the detail as required by the regulation: a.) Stronghold Digital Mining dba Scrubgrass Reclamation Co., LLC - Recordable injury cases on the OSHA Form 300, for calendar year2023, did not provide specific detail about the object and/or substance that caused the employee injury in Column F. This occurred on injury #5 on Calendar Year 2023 that occurred on 12/15/23. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: a.) Stronghold Digital Mining dba Scrubgrass Reclamation Co., LLC; 3rd floor loop for boiler - on or about 2/29/2024 and at times prior thereto, Employees periodically work on and in the boiler. There was no signage to inform employees the boiler was a permit required confined space. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
