29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) On or about March 21, 2024, an employee conducting window installation work on a house from a stack of decking material placed on the forks of an all terrain forklift were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately eleven feet onto dirt and building material on the front side of the structure.

29 CFR 1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) On or about March 21, 2024, an employee was working suspended from the forks of a JLG all terrain fork lift, while standing on a stack of decking material that was laying on but not secured to the forks or lifting carriage of the all terrain forklift. This exposed the employee to a fall hazard to the lower level of approximately 11 feet.
