29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about March 29, 2024, employees who entered Robot Cell 3 of the LALA Branded Products facility at 7122 J St, Omaha, Nebraska were exposed to struck-by and amputation hazards while performing production work within the caged enclosed FANUC Robot (Robot Cell #3). Employees are opening the interlocked gate to access the enclosure to insert flat cardboard as a separator within the stacked materials as well as manually aligning boxes that are not recognized by the automatic optic sensors. Hazards include but are not limited to: i. Employees exposed to amputation hazards related to the unguarded conveyor systems as they reach in and adjust and align boxes on the conveyor. ii. Employees exposed to struck-by hazards related to the robotic arm of the FANUC Robot while they are inside the enclosure
