29 CFR 1915.71(j)(2): Rails shall be of 2 x 4-inch lumber, flat bar or pipe. When used with rigid supports, taut wire or fiber rope of adequate strength may be used. If the distance between supports is more than 8 feet, rails shall be equivalent in strength to 2 x 4-inch lumber. Rails shall be firmly secured. Were exposed to hot work or chemicals, fiber rope rails shall not be used: Location: 1818 Brown Ave., Norfolk, Virginia 23504. Drydock # 2. On or about: 04/02/2024 a) Employees access to the drydocked fishing vessel Rappahannock was by way of a scaffolding stairway tower. Guardrail openings of the tower had wire rope guardrail supports that were not taut enough to guard from fall.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). Location: 1818 Brown Ave., Norfolk, Virginia 23504 Drydock #2. On or about: 04/02/2024 a) Employees worked installing scaffolding aboard the drydocked fishing vessel Rappahannock that had an unguarded deck edge. The Rappahannock deck guard rail was too low to guard from a fall.
