The flexible cord was dropped from the ceiling and plugged into the lift table cord. b.) Superior Tire & Rubber Corp. 1818 Pennsylvania Ave. West Warren, PA 16365 Rubber Molding Department, on or about 04/11/2024, and times prior thereto: A Flexible cord was used as a means of fixed wiring for the 110V lift table in the Rubber Mill Cracker Area. The flexible cord was wrapped around the building overhead pipework and bracing.

Superior Tire & Rubber Corp. 1818 Pennsylvania Ave. West Warren, PA 16365 Rubber Molding Department, on or about 04/11/2024, and times prior thereto: A Flexible cord was used as a means of fixed wiring for the 110V lift table in the Rubber Mill Cracker Area. The flexible cord was plugged into an overhead receptacle resulting in strain forces being applied to the connectors of the flexible cord and the lift table power cord. A means of strain relief was not used to prevent tension on the connectors.

The MC cable was plugged into an overhead outlet and was not supported at 6-foot intervals nor was supported within 12 inches of the receptacle.
