This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

The tasks included but were not limited to trouble shooting machine B7.  Employees who perform these tasks had not received authorized lockout tagout training. An employee sustained a thumb amputation and broken pointer finger to the left hand when the turbine rotated during troubleshooting as the hazardous energy inside the machine was not dissipated prior to service and maintenance work occurring.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
