29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  A.) Superior Tire & Rubber Corp., On or about 1/14/26 and times prior thereto: Employees did not lockout prior to performing troubleshooting activities on equipment including but not limited to the Mix Head on B7.  While troubleshooting an employee's hand was pulled in to the turbine resulting in a thumb amputation and broken pointer finger to the left hand.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.147(f)(3)(ii)(A): Primary responsibility was not vested in an authorized employee for a set number of employees:      A) Superior Tire & Rubber Corp., On or about 1/14/26, and times prior thereto: Three employees were performing service and maintenance tasks on the Mix Head of B7. A primary authorized employee was not designated to ensure that energy isolating devices were applied and to coordinate the group of employees who were involved in trouble shooting the machine.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Superior Tire & Rubber Corp., On or about 1/14/26 and times prior thereto:  Employees are periodically required to conduct servicing and maintenance on production equipment.  Lockout Tagout/Energy Control procedures were not developed for equipment including but limited to, the B7.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation    A.) Superior Tire & Rubber Corp., On or about 1/14/26 and times prior thereto: employees performed servicing and maintenance tasks on equipment including but not limited to the mix head on B7. The tasks included but were not limited to trouble shooting machine B7.  Employees who perform these tasks had not received authorized lockout tagout training. An employee sustained a thumb amputation and broken pointer finger to the left hand when the turbine rotated during troubleshooting as the hazardous energy inside the machine was not dissipated prior to service and maintenance work occurring.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
