29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  On or about April 23, 2024, Employee occupied scaffold vertical support poles were not set into base plates and mud sills or an adequate firm foundation, exposing the employees to fall hazards in the following instances (a-c):   a) Pump Jack Scaffolding on Southwest side of home.  b) Pump Jack Scaffolding on the Northeast side of home. c) Pole Scaffold on the Northwest side of the home.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:  On or about April 23, 2024, pump jack scaffold poles were not secured to the structure of the home by any means of bracing at the bottom of the upright poles, exposing employees to a fall hazard in the following instances (a-b):   a) Pump Jack Scaffolding on Eastern side of home. b) Pump Jack Scaffolding on the Western side of home.
