29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about November 12, 2024, on the west side of the construction site, employees used skill saws with the extension cord outer sheathing was pulled away. a. Mikita skill saw was hooked up to a power cord that had the outer sheathing pulled away; and b. DeWalt skill saw was hooked up to a power cord that had the outer sheathing pulled away.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about November 12, 2024, employees worked from two jack scaffolds from heights of more than 10-feet above the lower level without fall protection. Suarez Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1743346, citation number 01, item number 002 and was affirmed as a final order on May 24, 2024, with respect to a workplace located at 252 Jewelberry Street, Little Elm, Denton, Texas.
