29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. On or about April 8, 2024, at a workplace located at 206 Charles Street, Oconto, Wisconsin in the Laminations and Finishing Departments; the employer utilized fork extensions which affect the capacity and safe operation of forklifts without manufacturer's prior written approval. The Yale forklift, Model No. GLC050LXNVNE087, Serial No. A967V02140L utilized manufactured fork extensions to lift long pallets for core kit(s). Marine Technology Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.178(a)(4) which was contained in OSHA Inspection Number 1512596, Citation Number 1, Item Number 5a and was affirmed as a Final Order on June 20, 2021, with respect to a workplace located at 206 Charles Street, Oconto, Wisconsin.

29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about April 8, 2024, at a workplace located at 206 Charles Street, Oconto, Wisconsin in the Laminations and Finishing Departments; the employer did not request that the Yale LP forklift, Model No. GLC050LXNVAE087, Serial No. A967V02140L, be marked to identify the front-end attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered when the forklift utilized the manufactured fork extensions to lift elongated pallets. Marine Technology Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.178(a)(5) which was contained in OSHA Inspection Number 1512596, Citation Number 1, Item Number 5b and was affirmed as a Final Order on June 20, 2021, with respect to a workplace located at 206 Charles Street, Oconto, Wisconsin.
