29 CFR 1915.71(b)(5): Scaffolds shall be maintained in a safe and secure condition. Any component of the scaffold, which is broken, burned or otherwise defective shall be replaced.  On or about June 23, 2024, at location 605 N Third Avenue, Sturgeon Bay, Wisconsin: Employer failed to maintain Saflock System Scaffold in a safe and secure position where scaffold planks were loose and not secured resulting in an employee falling approximately 12 feet.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  On or about June 23, 2024, at location 605 N Third Avenue, Sturgeon Bay, Wisconsin: the employer neglected to inspect the Saflock Systems Scaffold following its assembly, exposing employees to fall hazards exceeding 10 feet.

29 CFR 1926.452(w)(2): Scaffold casters and wheels shall be locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold is used in a stationary manner.  On or about July 2, 2024, at location 605 N Third Avenue, Sturgeon Bay, Wisconsin: the employer failed to verify that the casters and wheels on the scaffold were locked as mandated while employees were working on the Saflock system scaffold.

29 CFR  1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform).  On or about July 2, 2024, at location 605 N Third Avenue, Sturgeon Bay, Wisconsin: The employer failed to ensure that the work platform had no gaps wider than one inch, exposing employees to tripping and falling hazards.
