29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) New residential construction located on 124 Phaeton Lane, Dawsonville, GA 30534, Roof: On or about 07/09/2024, workers were observed exposed to approximately 20-foot fall hazards from the 8/12 pitched roof in that the employer allowed employees to use rock climber type waist harnesses in place of full body harness.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) New residential construction located on 124 Phaeton Lane, Dawsonville, GA 30534, Roof: On or about 07/07/2024, the employer exposed employees to fall hazards of approximately 12-feet, 4-inches to the lower ground level, in that the ladder used to access the roof of the structure was not extended at least 3-feet above the upper landing surface or secured to prevent movement during use.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) New residential construction located on 124 Phaeton Lane, Dawsonville, GA 30534, Roof: On or about 07/09/2024, the employer exposed employees to struck-by hazards while operating pneumatic roofing nail guns, in that protective eyewear was not provided to employees operating these tools at 130 PSI.  Uriel Salto was previously cited for a violation of this occupation safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA Inspection Number 1652879, citation number 1, item number 1 and was affirmed as a final order date on 09/14/2023, with respect to a workplace located at 90 Stevens Cove Dr., Margaret, AL.

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.   a) New residential construction located at 124 Phaeton Lane, Dawsonville, GA 30534. On or about 07/09/2024, the employer exposed employees to potential struck-by hazards, in that the employer did not follow the manufacturer's specifications of the Transporter Platform Hoist as the wire cable it was manufactured with was replaced using a rope.
