Citation 2, Item 2 a Repeat Serious violation of 29 CFR 1926.503(c)(3) with an abatement date of September 10, 2024. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about July 19, 2024, at 10230 S Nicholson Rd, Oak Creek, WI 53154, employees installing a roof on an existing residential home with an approximately 7:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 8 feet 3 inches from the roof eve to the ground below on the left (North) side of the home. Zam Zam Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1622664, citation number 1, item number 1a and was affirmed as a final order on October 24, 2022, with respect to a workplace located at 2126 N 15th Street, Sheboygan, WI 53081.

(a) On or about July 19, 2024, at 10230 S Nicholson Rd, Oak Creek, WI 53154, the employer did not retrain employees that were not utilizing fall protection while installing a roof on an existing residential property with an approximately 7:12 pitch roof. This exposed employees to a fall hazard of approximately 8 feet 3 inches from roof to the ground below. Zam Zam Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 1622664, citation number 1, item number 1b and was affirmed as a final order on October 24, 2022, with respect to a workplace located at 2126 N 15th Street, Sheboygan, WI 53081.
