29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about September 4, 2025, employees conducting roofing work on the first story roof of the residential structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 8-feet, 9-inches up to 16 feet onto concrete on the East side of the first story roof over the garage .  (b) On or about September 4, 2025, employees conducting roofing work on the second story roof of the residential structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 19 feet to 24 feet onto concrete from the West end of the roof near the gabled ridge.  Zam Zam Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13)), which was contained in OSHA inspection number 1800805, citation number 2, item number 1, and was affirmed as a final order on April 11, 2025, with respect to a workplace located at 725 Small Farm Road, Mukwonago, WI 53149.  Zam Zam Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13)), which was contained in OSHA inspection number 1758242, citation number 1, item number 1, and was affirmed as a final order on December 12, 2024, with respect to a workplace located at 2931 E. Forest Hill Ave. Oak Creek, WI 53154.
